Job Description TITLE: Master Social Worker. LOCATION: Waukesha, WI 53188. DURATION: 13 WEEKS (POSSIBLE EXTENTION).
TIMINGS: 08:00 AM TO 4:30 PM MONDAY TO FRIDAY.

- Start at the Home Health office and travel with RN for patient visits within a 30-minute radius.

Requirements
Potential for Weekend On Call once every 3-4 weeks.
- Required Certifications to be Attached upon Submittal: BLS, License Verification. If offered would require Master’s degree or transcript.
- Minimum Experience: 2 years’ experience specialty, previous Home Health/Home Care experience required, a basic understanding of Home Care regulations.

None. Uses remote EPIC. • Unit Information (Bed size, Case type, Patient ratio).
