Overview

The Central Jersey Medical Center is a Federally Qualified Health Center in the state of NJ seeking a full time LCSW. Established in the year 2002, the CJMC has locations in Perth Amboy, Carteret, as well as 3 School Based Health Centers located in Newark, NJ. Our mission is to provide services to patients regardless of ability to pay, race, ethnicity, and cultural competence.

Responsibilities

Duties for this position include: Essential Functions:

  • Completion of a biopsychosocial assessments
  • Knowledge of community resources and appropriate referrals
  • Experience with the DSM5 and ICD10 for psychiatric diagnosis
  • Social Work consultation for the primary care providers including but not limited to ongoing case discussion and evaluation focusing on clinical goals and treatment plans.
  • Psychotherapeutic counseling, including resolution of symptoms involving mental disorders, psycho social stress, relationship problems or difficulties coping with social environment.
  • Client centered advocacy to bring about or influence change and improve quality of life.
  • Facilitate psychoeducational, therapeutic, and recovery-oriented group counseling sessions for individuals with mental health and/or substance use disorders’
  • Develop and implement evidence-based group curricula focused on coping skills, relapse prevention, emotional regulation, wellness, and recovery.
  • Monitor patient engagement, treatment progress, and clinical outcomes within both individual and group treatment settings.
  • Collaborate with interdisciplinary team members to coordinate comprehensive treatment interventions and ensure continuity of care.
  • Participate in the development and implementation of behavioral health programming, including group services, integrated care initiatives, and future program expansion efforts.
  • Contribute to quality improvement activities, outcome measurement initiatives, and program development projects.

Qualifications

  • A Master’s degree in Social Work, from an accredited college or university; experience working with troubled children and families; individual and group psychotherapy for children and adults.
  • Must possess a thorough knowledge of principles and techniques of youth and family counseling; working knowledge of human services system; ability to conduct and facilitate meetings; ability to maintain effective working relationship with other social service providers; ability to communicate effectively both orally and in writing.
  • Experience with electronic medical records, email, work and excel preferred.
  • Experience facilitating behavioral health, psychoeducational, substance use disorder, recovery-oriented, or process groups
  • Experience working within Intensive Outpatient Programs (IOP), Partial Hospitalization Programs (PHP), or other structured behavioral health treatment settings preferred.
  • Knowledge of evidence-based treatment modalities including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing (MI), Trauma-Informed Care, and Relapse Prevention.
  • Experience treating individuals with co-occurring mental health and substance use disorders preferred.
  • Experience developing, coordinating, or facilitating structured behavioral health programming strongly preferred.
  • Bilingual English/Spanish preferred.
  • Preferably able to speak, read and write Spanish and English.
  • Must possess a NJ license as a Clinical Social Worker (LCSW)
