Primary Care Physician

Company Description A 42-year-old Independent, Physician Driven, and Professionally Managed Primary Care group located in greater Jacksonville is looking for a full-time Physician to join the team! We are the   second largest primary care group   in North Florida. The candidate will be   inheriting   an Existing Patient Panel from a Physician who is retiring.

Responsibilities

Include:

• Primary Care work with a mixed patient population
• Hours of Operation – 8 am to 5 pm, M-F   with flexibility
• Average patient load 18-28 patients per day
• Required to be available for call coverage once every 10 weeks   (1st   call goes to Extenders so Physician call if any, is very light)
• Care Team Model in place so Extenders do most of the work ( we work with Top of License MA’s and Medical Staff)
