Job Summary

Act as the primary point of contact for assigned patients across all enrolled Care Management Programs.o

  • Coordinate with support staff for tasks such as referrals, DME, labs, and diagnostic orders.o Creation of care planso Identification of Gaps in cares.o
  • Track patient progress for all programs (APCM, CHI, PIN, BHI) and update records in eClinicalWorks (eCW)/Ecares.o
  • Monitor adherence to care plans and ensure timely follow-ups.o Proactively identify issues across programs and escalate unresolved or complex cases to the care manager.o
  • Communicate with patients and caregivers to provide updates on all programs.

Responsibilities

  • Maintain accurate documentation of services provided, including time tracking for program billing.o
  • Collaborate with support staff to ensure smooth task execution.o
  • Participate in quality reviews and team meetings to share insights

Qualifications

Experience:1. 1-2 years of direct patient care experience2. Experience in Care Coordination or Case Management3.

Experience with Remote Patient Monitoring4. Familiarity with EHRRate: $20.00
