Nurse Practitioner – Nursing Home Rounds (Pain Management)

About the Role

We are seeking a   Nurse Practitioner   to provide   on-site rounding services in nursing homes/skilled nursing facilities , with a primary focus on treating a combination   of acute and chronic pain syndromes. This role is ideal for NPs who enjoy   independent practice , value   consistent schedules , and are comfortable managing   complex, medically fragile populations . What You’ll Do

• Conduct regular   on-site rounds   in assigned facilities
• Evaluate and manage   acute and chronic pain conditions
• Prescribe and manage medications, including   controlled substances
• Coordinate care with facility staff (PT’s, RNs, DONs, administrators)
• Complete timely EMR documentation Patient Population
• Acute rehabilitation and long-term care residents
• Geriatric patients with multiple comorbidities
• Chronic pain, mobility-related pain, and dementia-associated pain syndromes Schedule & Work Environment
• Primarily   weekday daytime hours
• No hospital shifts
• Travel between facilities is required in some cases
• Predictable patient census and rounding schedules What We’re Looking For
• Active   NP license   in the practicing state
• DEA license   (or ability to obtain)
• Experience or strong interest in:
• Geriatrics
• Long-term care
• Pain management
• Comfortable working independently and traveling locally
