Overview

Join our Home Health team as a Licensed Practical Nurse (LPN), delivering compassionate, high-quality care to patients in the comfort of their homes. Working under the supervision of an RN, you’ll play a key role in supporting patient outcomes, coordinating care, and empowering patients and families through education.

Key Responsibilities

• Deliver skilled nursing care in accordance with individualized care plans under RN supervision
• Monitor patient conditions, recognize changes, and communicate updates to the care team
• Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
• Provide patient and family education on disease management, treatments, medications, and safety Accurately document care to support clinical, regulatory, and reimbursement requirements
• Participate in case conferences, team meetings, and ongoing care planning Promote patient independence and engagement through education and support
• Maintain compliance with organizational policies and quality/safety standards

Required Qualifications

Active LPN license in the State of Illinois (or eligible) CPR Certification Pharmacology certification Minimum 2 years of Med/Surg clinical experience Valid driver’s license and reliable transportation Preferred

• Experience Prior home health experience
• Familiarity with Medicare/Medicaid home health guidelines Skills & Strengths
• Strong clinical assessment and critical thinking skills
• Excellent communication and documentation abilities
• Ability to work independently and manage time effectively
• Comfortable using EMR systems and basic Microsoft Office tools
• Strong interpersonal skills with a patient-centered approach
