Overview
Join a mission-driven hospice team serving the Keene, Peterborough, and Charlestown, NH communities. As a Care Transitions Specialist, you'll build relationships with hospitals and healthcare facilities, support patients and families during critical transitions, and help ensure timely access to hospice care.
What You'll Do
- Build relationships with hospitals, assisted living facilities, and skilled nursing facilities
- Identify patients who may benefit from hospice services
- Coordinate care plans with patients, families, nurses, physicians, and discharge planners
- Educate referral partners and community members about hospice services
- Support timely admissions and smooth transitions of care
- Collaborate with the hospice team to improve patient outcomes
- Track referral activity, goals, and outcomes

Requirements
- Bachelor’s degree in Nursing or Social Work
- At least 3 years of experience, preferably in home health, hospice, acute care, hospital, or community health settings.
